Welcome to the Referral Process

Current Foster Youth:

If you are a current foster youth age 16-18, please contact your social worker and ask him/her to fill out a referral form (pdf) and fax it to 510-667-7639. As a team, you and your social worker can gather all the necessary information for the application process.

If you don’t have access to your social worker, please complete the referral form and fax it in yourself.

If you are a former foster youth age 19-24, please click on the referral form and fax it to 510-667-7639.

 
Child Care Professionals (child welfare workers, foster parents, CASAs, group home providers, and other support persons):

If you are working with a current or former foster youth in need of employment or other youth development services, Project HOPE can be a connection point to these services. Please consult with the youth before referring them to Project HOPE. To make a referral, please click on the referral form hyperlink above.

Please note that having filled out the referral form and having faxed it to Project HOPE does not mean automatic enrollment for services. As described earlier, Project HOPE works to connect current and former foster youth to a network of employment training services in Alameda County, including the City of Oakland.  These programs have their own application and assessment processes and enrollment decisions are determined by individual programs.
